APRIL

Officer List – Spring Update - Provide update of new officer information. Email the form to executivedirector@phichitheta.org

Founders’ Day Report – Submit details of the chapter’s Founders’ Day Activities using the Event Summary Form. Email the form to executivedirector@phichitheta.org

Submit Chapter Bylaws – The chapter will email an electronic copy of the most current version of their chapter bylaws to executivedirector@phichitheta.org

Spring Initiation Form & Fees – Form attached. This should be submitted to Nationals 14 days prior to the ceremony in order to allow time for me to get the member cards and pins ready and mailed to the chapter to present to the new member during the Initiation Ceremony. Failure to do this cheats the new member out of having the complete experience that they deserve. An electronic version of this form she be emailed to executivedirector@phichitheta.org and a hard copy should be mailed in along with the fees to the National Office (address on the form).

MAY

Hold Officer Training/Planning Meeting - Conduct officer training with outgoing and incoming officers. Complete the Event Summary Form to provide details of the event & email the form to executivedirector@phichitheta.org

Signed Officer Instructions – Using the list of duties that the chapter has established for the officers (in the chapter’s bylaws, standing rules, etc) have the incoming officers sign that they understand the duties and responsibilities of that office and are agreeing to fulfill those requirements. Email scanned copy to executivedirector@phichitheta.org or mail hard copy to the National Office (1508 E. Beltline Road, Suite 104, Carrollton, TX 75006)

All Scholarship Applications – Applications can be found on the National Website under Foundations. No reporting is necessary. The Foundation will confirm that the applicant is an “active” member and confirmation will be made and award points given at that time.

Summer Officer Address Update - Use Summer Officer Update Form – this contact information is for the new/incoming officers.

Professional Development Report – If the chapter held any professional development programs for the semester, please report using the Professional Event Summary Form in order to receive award credits.

Fundraising Report – If the chapter held any fundraising events during the semester, please report using the Fundraising Summary in order to receive award credits.

Spring Graduates – Please use the 2008-2009 Graduate List to report all graduates. The graduates will receive graduate information and it will allow Nationals to update the database for the chapter.

Hold One Alumni Activity – If the chapter held an alumni activity during the semester, please report using the Event Summary Form in order to receive award credits.
